CANCER TREATMENT WITH COORDINATE HEALTH 
THE PROCESS 

Coordinate Health develops real relationships with providers and payers to identify and support newly diagnosed cancer patients. Once a primary care or specialty physician refers a patient and their family, a qualified Care Adviser begins the process of advocating for them. Among the services: 

  • A local, trusted adviser supports that patient and family from the time of diagnosis throughout the continuum of care.

  • Patients receive education about and access to all available treatment options, as well as opportunities for additional diagnostics.

  • A neutral third-party care-management platform incorporates all treatment and care options. 

• A Care Adviser becomes a single point of communication. That means coordinating all external resources, including the patient’s financial, nutritional, emotional, spiritual and holistic needs. It also means coordinating consistent information among all members of the patient’s treatment team.

• An independent financial adviser assists the family with benefits management. 

Coordinate Health engages at the point of diagnosis with a team-based patient centered approach.  We focus on giving patients and their support systems the opportunity to spend quality time together-while we make sure their most pressing needs are met.  We provide the support to make sure prescriptions are filled, appointments are scheduled, test results are provided, patients understand their benefits, and confusing or conflicting medical information is explained.
