Us

We foster a personal relationship with patients and their support systems. And we equip them with the appropriate information, care coordination and benefit management that helps them make more informed treatment decisions. Our goal is to improve the patient experience by helping to reduce their exposure to unnecessary testing and treatments, while eliminating excessive costs to their health plans.

By committing to patient-centered care coordination and benefit management from the time cancer
is suspected or diagnosed, our Care Advisers are able to educate, offer guidance and coordinate multiple services. Ultimately, Coordinate Health provides the connections to appropriate treatment — with qualified providers at the right site of care to meet the goals of the patient and their health plan. 

THE NEED 

The statistics are sobering: In 2016 alone, more than 1.6 million people will be diagnosed with cancer. The cost of cancer care continues to rise, with the National Cancer Institute projecting it at nearly $158 billion per year by 2020.

More than 80% of all newly diagnosed patients are 50 or older, and they have little knowledge or understanding of the complicated world of cancer treatment. Not surprisingly, a 2015 Kaiser Health study showed that more than a third of these patients receive inappropriate or unnecessary testing or treatment. 

  • Is complicated and often ineffective at developing true patient-centered care.

  • Is susceptible to a physician’s emotional, intellectual or financial bias — with no regard to industry standards or the patient’s quality of life.

  • Offers the patient no accessible, comprehensive review of treatment options and alternatives.

  • Often involves unnecessary treatment and duplication of tests

  • Does not support active care management and coordination of resources

Their stress is compounded when prescriptions can’t be filled, benefit coverage is confusing or various members of the medical team provide conflicting information.

Amy Radcliff has focused her career in healthcare around patient-centered solutions.

As President and COO of e+cancercare, a leading operator of comprehensive outpatient cancer centers, Amy initiated the development and implementation of a patient centered care coordination platform to support patients from diagnosis to survivorship/end of life care.  She also led the development of Vanguard Health System’s (now Tenet Health) first comprehensive community cancer center. This Center served as the flagship cancer center for the organization and the pilot for using patient-centered care coordination to integrate care between a closed health plan, independent and employed providers.  Prior to her healthcare career, Amy led teams focused on solving complex data and IT challenges in the telecommunications industry.
